ACCOUNTABILITY OBJECTIVE:

This position is accountable for the planning, coordination and delivery of quality health care and services, assuring compliance with CareGivers standards and federal/state and local guidelines.

 

NATURE AND SCOPE:

This position reports to the Administrator, or Director of Clinical Services.  Reporting to this position are the staff engaged in direct delivery of home care services.

The principal focus of the incumbents’ activity is to insure that clients served by CareGivers receive the highest quality care through the ongoing evaluation of client needs and employee performance.

This position is responsible for the performance or delegation of initial evaluation of client needs, writing a realistic goal oriented plan of care, and supervising its implementation.  The incumbent is also responsible for supervising the implementation of the physicians’ plan of treatment.

The incumbent maintains responsibility for coordinating the total care plan with appropriate resources, conducting periodic reassessments of client needs, revising the plan of care, and monitoring employee performance.

In monitoring the skills and services of field staff, the incumbent regularly reviews the client notes of the field staff and engages incase review with the staff.  The incumbent is responsible for the maintenance of complete, current, accurate client records.  The incumbent also invites feedback through client and family contact.

In the absence of a Director of Clinical Services, this position may participate in the selection and orientation of new health care staff and the continuing evaluation of staff in the field.  In conjunction  with the Administrator and Director of Clinical Services, the incumbent will participate in the completion of written evaluations of home care employees and the development of recommendations for promotion, probation, and termination.

Maintaining ongoing communication with the staff is also a responsibility of this position.  The incumbent participates in staff conferences, assisting in the establishment of short and long range goals. The incumbent is also responsible for interpreting nursing and CareGivers policies and procedures to both nursing and non-nursing personnel.

This position may be responsible for maintaining liaison with referral sources and community resources, and participating in local professional organizations and activities.

The incumbent is also responsible for maintaining effective communications between CareGivers and the client it serves.  Additionally, the incumbent must be available for consultation with any third party bearing financial responsibility for the client.  The incumbent may serve on such committees as the Advisory/Quality Improvement/Advisory Committee and Peer Review Committee.

These and other responsibilities are assigned in response to individual office needs.

 

QUALIFICATIONS:

The incumbent must be a registered nurse, currently licensed/registered in New York State, with at least one year experience as a professional nurse within the last two years.  Six months of supervisory experience or one year of experience in home care is preferred.  Current certification in Cardiopulmonary Resuscitation (CPR) is required. Knowledge of home health regulations is preferred.  The incumbent must possess strong communication skills and the ability to maintain a flexible schedule.

The responsibility of this position requires that the incumbent be able to provide transportation. This position requires the completion of health examination and required tests/immunizations.  The completion of annual health assessment thereafter to certify that the incumbent is free of communicable disease and physically capable of performing assigned duty.

 

PRINCIPAL ACCOUNTABILITY:

  1. Maintain ongoing assessment in case supervision of CareGivers clients, assuring an accurate evaluation of patient needs.
  2. Responsible for supervising and monitoring the performance and skills of field employees, assuring the delivery of quality health care services.
  3. Maintain current and accurate client records in accordance with CareGivers policies and procedures.
  4. May assist in the selection, orientation, development and evaluation of home health care employees, assuring compliance with legal and CareGivers standards.
  5. Maintain effective liaison with agency personnel and community health organizations.
