ACCOUNTABILITY OBJECTIVE:

This position is accountable for providing personal care and designated health related services designated to maintain the client’s physical and emotional well-being in compliance with CareGivers policies/procedures and standards, including all federal, state and local regulations.

NATURE AND SCOPE:

This position reports to a registered nurse who may be a Clinical Supervisor or case Administrator, or to professional contracting Agency Personnel.  In some clinical assignments, the incumbent may report to a registered therapist in accordance with CareGivers and regulatory policy.

The principal focus of the incumbent’s activities is to provide personal care services according to written directions prepared by a registered nurse, physical therapist or speech pathologist.  These services are carried out under the supervision of the appropriate professional.  This position is responsible for providing personal care, rehabilitative and supportive services required in the Plan of Treatment and the Plan of Care which includes, but is not limited to assisting the client in activities of daily living, planning and preparing meals, and maintaining a clean and safe environment.  The incumbent may assist the client with medications which are ordinarily self administered.

The incumbent is responsible for assisting the registered nurse in the implementations of a realistic, goal oriented Plan of Care.  The incumbent observes and reports significant changes in the status of client and/or family. This position accurately records his/her observations and completed tasks.  This position is responsible for performing assigned homemaking tasks.

The incumbent is responsible for preparing and submitting timely documentation of care and time slips in accordance with CareGivers policy.  The incumbent may participate in case conferences.  The incumbent must attend required inservice hours to maintain certification.

QUALIFICATIONS:

The incumbent must have completed an approved New York State Department of Health Homemaker/Home Health Aide Training Program; or successful completion of the competency testing if he/she meets New York State criteria for same.  The incumbent must be physically capable of performing job related duties and be able to read, write, understand and carry out directions.  The incumbent must be able to provide or have access to a reliable means of transportation.  This position requires the completion of a health examination and immunization prior to employment and periodically thereafter to certify that the incumbent is free of communicable disease and physically capable of performing assigned duties, including the following:

  • lifting and turning of patients in bed or chair*
  • transferring patients from bed to chair, chair to bed, etc.*
  • giving a complete bed bath*
  • assist with bathing at sink, tub, shower*
  • bedmaking, occupied or unoccupied bed*
  • assist with toileting*
  • patient transfer with use of equipment, e.g., mechanical lift, slide board*
  • assist with ambulation*
  • assist with feeding*
  • light housekeeping activities such as vacuuming, sweeping, dusting, mopping, washing dishes, laundry, ironing, maintaining cleanliness of patient’s environment, shopping for patient

PRINCIPAL ACCOUNTABILITIES:

  2. Observe client and communicate any change in status.
  3. Prepare and submit accurate and timely documentation of services provided to clients as required by CareGivers policy and government regulations.
