ACCOUNTABILITY OBJECTIVE:

This position is accountable for providing nursing services in compliance with CareGivers policies/procedures and government and professional standards.

 

NATURE AND SCOPE:

This position reports to the Registered Nurse or Clinical Supervisor or reports to the Director of Clinical Services or Administrator.

This position is responsible for providing client health care services primarily on a part-time, intermittent basis.  Services are provided under the direction of a registered nurse.  Services include preventive, supportive and rehabilitative activities in accordance with the Plan of Care and the Plan of Treatment. State licensure provisions may limit services the incumbent may provide.

The incumbent participates in the development and periodic re-evaluation of the Plan of Care.  The incumbent is responsible for observing, recording and reporting changes in the client’s condition and responses to treatment.  The incumbent prepares and submits timely documentation as required by CareGivers policy.  The incumbent participates in case conferences and inservice education programs as requested.

The incumbent is also responsible for additional duties assigned in response to office need.

 

QUALIFICATIONS:

The incumbent must be a Licensed Practical Nurse, currently licensed in New York State with at least six months experience as a Licensed Practical Nurse and/or competent in knowledge and skills based on successfully completion of the pre-employment nursing test and demonstration of skills. Must be competent in all skills required on assignment following case orientation and on the job training. Current certification in Cardiopulmonary Resuscitation (CPR) is required.  The incumbent must be able to

provide a reliable means of transportation and may be required to possess a valid drivers license.  The incumbent must be physically capable of performing job related duties and able to function with minimum supervision.  This position requires the completion of a health examination and immunization prior to placement on a case and completion of an annual health assessment thereafter to certify that the incumbent is free of communicable disease and physically capable of performing assigned duties, including the following:

  • lifting and turning of patients in bed or chair*
  • transferring patients from bed to chair, chair to bed, etc.*
  • giving a complete bed bath*
  • assist with bathing at sink, tub, shower*
  • bedmaking, occupied or unoccupied bed*
  • assist with toileting*
  • patient transfer with use of equipment, e.g., mechanical lift, slide board*
  • assist with ambulation*
  • assist with feeding*
  • light housekeeping activities such as vacuuming, sweeping, dusting, mopping, washing dishes, laundry, ironing, maintaining cleanliness of patient’s environment, shopping for patient

PRINCIPAL ACCOUNTABILITY:

  1. Provide direct client care in accordance with CareGivers policies and procedures, government and professional standards, the client’s Plan of Care and the Physician’s Plan of Treatment.
  2. Participate in the development and the periodic re-evaluation of the Plan of Care.
  3. Prepare and submit accurate and timely documentation of service provided to clients as required by CareGivers policy, procedure and government regulations.
